Tips for submitting a successful referral application

  • Include the full legal name, date of birth, and email addresses of all adults listed on the referral.
  • Parent/guardian and patient information should only be entered once. Please use the Additional Guest Information section to provide information for other adults or children.
  • If the location closest to the treatment facility is at full capacity, please indicate if the family is willing to stay at the other location.
  • Include the patient’s diagnosis whenever possible. This helps us prioritize room requests during periods of high occupancy.
  • If any family members do not have a Social Security Number, please indicate this in the notes section.

After the referral

For families

After a referral is submitted, we will contact all family members age 18 and older to begin the background check process and provide next steps.  Families should regularly check emails to make sure they don’t miss any outreach.

Families will be notified of House availability 1-3 days before the requested arrival date.

If there is no availability at one of the Houses, families will be automatically added to a waitlist.

For referral partners

Our staff will remain in communication with referral partners as needed to support families throughout their stay.
